  • Professional School Counselors/ PGCPS Employees:

    Students who are unable to access their virtual classroom due to a medical condition will be considered for additional support coordinated through the Office of Home and Hospital Teaching. With consideration to federal laws that require the provision of instruction in the least restrictive environment, all efforts will be made to support student participation with his/her assigned classroom using accommodations or modifications to address any medical, instructional and accessibility needs.

    Referral Form for PGCPS Employee:   Home and Hospital Teaching Referral Form (DSS21)  

    Medical Verification Forms (DSS-22/22A): 

    Forms should NOT be completed more than 15 days prior to the date the doctor indicates HHT services should begin.
