NEW STUDENT REGISTRATION
WHO MAY REGISTER A STUDENT
Only a parent or court-appointed guardian may register a student in a Prince George’s County Public Schools. If a student is residing with anyone else, they must either obtain legal court-appointed guardianship or apply for kinship care approval. For additional information, call the Department of Pupil Accounting and School Boundaries at 301-952-6302. A child may not be registered on a notarized statement giving guardianship. Please note that it is not necessary to apply for court-appointed guardianship to apply for a kinship care approval.
Please bring the following documents at the time of registration:
Proof of birthdate - preferably the child's birth certificate. The child’s name, child’s birth date and parent’s name must be listed on the document provided. The following may serve as proof of birthdate:
- Birth Certificate or Maryland Certification of Birth Card
- Physician’s Certificate
- Baptismal or Church Certificate
- Parent’s Affidavit (issued by the International Student Guidance Office only)
- Family Bible
- Passport/Visa/Naturalization documents
- Birth Registration
- Other
Any student that is new to our county since September, 1998, must have a copy of the birth verification attached to the Student Registration Form, PA14.
Legal proof of residence - The following may serve as proof of residence:
- Settlement Papers
- Rental Receipt
- Tax Assessment
- W-2 Form
- Lease Agreement (A copy is acceptable. Most Apt. complexes do not provide an original lease to the lease holder.)
- Deed of Property
- Mortgage Statement
Student transfer card - or last report card from previous school, for students entering from another school system. Please note that the parent/guardian must bring the withdrawal packet from the previous school. Under state law, an "Affidavit of Disclosure" must be completed at the time of registration. This form is a verification of residence for newly arrived or initial enrollments. Anyone found guility of giving false information on this form may be fined as much as $250 in penalities.
Record of immunization -The Maryland State Department of Health and Mental Hygiene has regulations which require certain immunizations before children can be admitted to school. Please view the immunization requirements for the 2011 - 2012 school year to ensure your child's compliance with county policy.
