 Registration Information

Only a parent/court appointed guardian may register a student.

You must have the following documents to complete a registration:

*Birth Certificate

Parents name must be on it.

*Shot/Immunization Record 

Must have physician signature/stamp affixed and include Lead Testing

*Proof of Residence:

If documentation is older than 30 days you must also provide 2 utility bills/business mail. 

Deed, Settlement Papers, Property Tax Bill, Mortgage Statement, Lease

*Picture ID

Of the Parent/Court Appointed Guardian

*Proof of Income (Pre-K Only)

Pay stubs, W-2, Unemployment Benefit Letter

**Please contact the school if the property is not in your name for additional information.

Please contact the school and allow 24-48 hours to process the withdrawal.

 You will receive the required documents needed to complete the

registration process at the new school!

need members to support the parent group.

Please stop in the main office for a membership form.

Dues are $10.00 per year.

Meetings are held 2nd or 3rd Wednesday of each month @ 6PM
