Ordering Guide

All Offices and Schools should be using the Oracle Account number based G-form. Make sure you are using form revision # P-12 (located at the bottom of the form).

ALL SHADED AREAS MUST BE COMPLETED
If you are not sure of how to fill out one or more of the shaded areas, please feel free to contact the office at 301-952-6295 for assistance. Be sure to include the following necessary information:

• Complete Oracle Code
• Distribution List
• Full Name (no nickname, initials, or last name only)
• Contact Number
• Color (if applicable)
• Final Print Size

Currently, our method of submission is the printed G-form.  In the near future, we expect to have electronic submission in place.
