To Enroll your child, you will need:   

Head Start Program: 

• Application (Competed & Signed) 
• Proof of Child's age (Birth Certificate - Child must be between 3 & 4 yrs of age) 
• Proof of current income of Parents/Guardians of child
  (Submit 4 current pay stubs; 2010 1040 tax form (1st two pages); public assistance letter; Social Security Income; unemployment verification; or verification of your wages signed by your employer.) 

If your child is accepted, you will be required to submit a health package and provide proof of residency, for your child to be considered for enrollment into Head Start.

Kindergarten thru 6th Grade:

• Birth Certificate (With the Seal)
  (Birth Registrations & Birth Certificate Cards are NOT acceptable)
• Social Security Card
• Valid ID (Of Registering Parent/Guardian)
• Proof of Residency
  Current Lease or Deed (With Deed need a Current Utility Bill)
  Current Rental Receipt on Company's Letter Head
  Current Shared Housing Disclosure Letter (Only if Registering Parent Name is not on Lease)
• Shot Records (Must Be Current)
• Last Report Card                                                                                            

You Child CAN NOT Be Enrolled Without These Items!
