Important Notice to parents If your child requires any medication to be given during school hours, a medication authorization form must be completed and signed by your child’s doctor and by a parent or guardian. This form must be completed annually. Do not send inhalers or other medication to school without having proper medication forms completed. School staff cannot administer medication to your child until we have completed forms. You may come to school to administer your child’s medication while waiting for these forms to be completed. Please notify me at the beginning of the school about any health concerns you have for your child and any special needs he/she may have during school hours. If there are any changes during the year, please call me.
