    Becoming a PTSA Member/Renewing Membership:

    To become a member or renew your membership, you should:

    1. Fill out the attached CHS PTSA membership form. Please include your email address and home address for future information.

    2. Write a check or money order to “CHS PTSA”. Membership dues are $12 for adults and $5.00 for students. Check the membership form for possible discounts.

    3. Give your completed membership form and payment to Mrs. Yates in the front office or give your check to our treasurer during our monthly PTSA meeting (3rd Tuesday of each month).

    4. Wait for your electronic notification that will be sent to the e-mail address you listed on the membership form. This notification will confirm receipt of your payment.

    5. Wait for you membership card, which will be mailed to your home address OR given to you at the general membership meeting. Please remember to activate your card to receive your PTSA discount privileges.
