In connection with the Affordable Care Act and healthcare reform, PGCPS provided to employees and retirees a new healthcare information Form 1095-C, Employer-Provided Health Insurance Offer and Coverage Statement, which included information regarding whether medical insurance coverage was offered and maintained by employees/retirees and their dependents through PGCPS' medical Plan during 2015.  While the information on the Form 1095-C may useful in preparing your 2015 federal tax return, it is not required and does not need to be attached to your tax return.  The form should be kept with your 2015 tax records.
