    Supplemental Life Insurance Plans 

    (View Summary)

    • The PGCPS Supplemental Life plans are administered by The Hartford
    • Newly hired employees may enroll within 30 days from their date of hire using Oracle Self Service
    • Employees enrolling during open enrollment must provide Evidence of Insurability
    • Supplemental Life Insurance is available to employees and their dependents (spouse & child(ren))
    • Employee coverage amounts are in increments of $50,000 to a maximum of $500,000 not to exceed 5 times annual salary
    • Dependent Spouse coverage amounts are in increments of $5,000 to a maximum of $50,000
    • Dependent Child coverage amounts are in increments of $2,000 to a maximum of $10,000
    • Employees who enroll into the Dependent Life plans must enroll in the Employee Optional Life plan, beneficiaries are added using Oracle Self Service
    • Premiums are deducted on an after-tax basis

    Long Term Disability Insurance 

     (View Summary)

    • The PGCPS Long Term Disability plan is administered by The Hartford
    • Newly hired employees may enroll within 30 days from their date of hire using Oracle Self Service
    • Employees enrolling during open enrollment must provide Evidence of Insurability
    • The plan provides income protection up to 60% of your monthly salary to a maximum of $5,000
    • Benefits are paid for as long as you remain disabled, or until you reach your Social Security Normal Retirement Age (as stated in the 1983 revision of the United States Social Security Act), whichever is sooner. If your disability occurs at age 63 or above, your payments may be reduced.
    • Waiting period; 180 days and/or exhaustion of Sick Leave Bank benefits
    •  Premiums are deducted on an after-tax basis
