2012 - 2013 REGISTRATION	PTO OFFICERS
Required documentation for Registration: Birth certification with Mother's Name on it Immunization Records Proof of Residence Social Security Card PreK Registration also requires you to provide 2 current paystubs! MIDDLE SCHOOL TRANSITION If your child will be attending Middle School in the fall you must go to the elementary school before the end of this school year to complete a new registration form to be delivered to the Middle School with their other records. Failure to complete this form may delay scheduling for the fall semester. This is also a good time to update your address, if necesary, remembering that a new Proof of Residence is required with this update. If you do not receive a 2012-2013 schedule for your child before June 8th you will have to go to the Middle School to complete a new registration.
